Delta Dental Updates (1/1/2010)

Effective January 1, 2010, your Dental Benefit Plan Summary has been amended:

1. Benefit Maximums:  The Program pays up to a maximum of $1,750.00 for each Covered Person per Coverage Year subject to the coverage percentages identified above. Benefit Maximums may not be carried over to future coverage years.

2. Single Tooth Implant Body, Abutment and Crown:  Covered 1 time per 5-year period for covered persons age 16 and over. Coverage includes only the single surgical placement of the implant body, implant abutment and implant/ abutment supported crown.

3. New Exclusions Language: (Revised) Any material grafted onto bone or soft tissue, including procedures necessary for guided tissue…(Deleted) The replacement of an existing partial denture with a bridge regeneration.

Q: How do I enroll in Delta Dental?

A:   Cindy Pince, from Council 5, will contact you within 30 - 60 days after your hire date. If she hasn’t contacted you within 60 days of your hire date, then call her at 651-287-0564 and leave her a message with your name, contact information (including mail code), employee number, and your hire date. 

Q: Who is covered by my Delta Dental benefit?  Is this only a benefit for Full Members of the Union?

A:  You (employee), your spouse, and your dependents are covered. There is no additional cost to add your spouse and your dependents to the plan during enrollment or open enrollment in November. If you are single, you can add your spouse when you get married at no additional cost. If you don’t have dependents, but have children and/or adopt at a later time, you can add your dependents at no additional cost. All members of the Union, whether you are a Full Member or a Fair Share Fee Payer, are entitled to enroll in Delta Dental.  

Q: When is open enrollment?

A:   In November of every year. If a special circumstance occurs, (i.e. marriage, divorce, birth or adoption of a child, child leaves the home, child graduates from high school but doesn’t wish to attend college), then you can add or remove family members any time during the course of your enrollment with Delta Dental. You can call Cindy Pince at 651-287-0564 and ask for a “Delta Dental Maintenance Form”.  

Q: Can I enroll in Delta Dental on line via the Delta Dental web site?

A:   At this time, Local 34 is not utilizing the on line Delta Dental form from the web site. Please wait for Cindy Pince to contact you or you can contact her if you haven’t received a Delta Dental packet and it is 60 days since your hire date. 

Q: What type of plan am I enrolled in as a Delta Dental member?  When does my plan take effect?

A:  Currently the Union enrolls in a “Comprehensive Enhanced” plan that utilizes both “Preferred Option” (i.e. smaller network of providers but maximum payment for services) and “Premiere Option” (i.e. larger network of providers but less than maximum payment for services). Your plan will take effect on the first day of the month after one full calendar month of employment (Example: Hire date is January 30th, your Delta coverage begins on March 1st.)  Please contact Delta before you attend any appointments to assure that you are covered on Delta. If you find out that you are not covered, then cancel your dental appointment and reschedule it to assure that your benefit will pay for services rendered by your dental provider.
